Operating within the PICS Team of Care
Managers, Social Workers, Community Nursing Staff, to assess
the needs of vulnerable adults.
The successful post holder will work across health, social
and voluntary services to maximise the quality of life for
these individuals. Focus will be on helping to prevent
unnecessary hospital admissions by providing clinical and
social support within the home where possible.
You will be working to promote a person centred approach and
actively engage local people in developing independence,
choice and control in their care.
You will also be working proactively with the patient, GPs,
Community Nursing Staff, Social Services, Intermediate Care
Teams, Specialist Teams, Voluntary Sector, Secondary Care
and Community workers.
You will need:

A professional registration
qualification in Social Work or allied profession.

Experience of working with adults
(including those with mental health issues) and/or their
carers in a health or social care environment.

A broad working knowledge of Health,
Social And Voluntary Care Services.

Effective negotiation ability at all
levels.
